I did and it was when I was nine years old.

Being close to death in the sense of anesthesia is not "almost" dying. "Almost" dying includes the possibility that you actually can die, and although that has happened in the past, it is very, very rare. Saying you almost died while under anesthesia is like saying you almost died because you were in a lightning storm or you drove your car to work.
Yes. Several times. Fortunately not from some illness or severe accident, but rather from my craziness or naivety. I was almost trampled by a speeding Mini Couper and it would be my fault for crossing the road in front of the bus... I was recued from a small "island" (I couldn't swim) at like 1 or 2 Kms from the main beach, because it wasn't deep before but it turned out to be when I was returning... I adventured or ventured too much into the distance over the rocks near the sea (which was not being very friendly) and it was really hard to come back... I don't know, there were more times.
When I look back I think I was so lucky... that now I value "little things" in life more than most people do (apparently). And I just can't fucking quit smoking...
The only time that really comes to mind is when I was in college and on my way to work during an ice storm. I walked a different way that day than usual, and as I crossed the street, at the corner where I would have been had I crossed the side street first as I normally did, a giant branch fell smack down on the sidewalk.
No telling if it could have been me or not, but it scared me a lot.
Oh, and one other time, my house was hit by lightning and I was about ten feet away.
I don't really qualify either of these, because to have almost died from them I would have had to been struck and survived. But still, scary sh*t.

Several times, SILLY TIMES, but I still won't forget them.
I was once in Oregon, scaling a rock face in the ocean. I was a small child, and instead of trying to find away a pit with jagged rocks at the 20ft bottom, I jumped the pit.
And barely made it.
I didn't even realize until hours later that I could have killed myself. I had found it fun when I did it.
Second time... I was out cutting wood. My brother was using a spliting maul to chop the wood easily. He brought another blow to the wood down, and the maul glanced off the wood, and the head of the maul pass within an inch of my forehead.
If it has hit me, my head would have been cleaved open.
Those are the only 2 times I can remember off the top of my head. But yes, it almost happened both times. I was lucky both times to.
I have had a few near death experiences in my life and trust me they were frightening. But what has hit me the hardest is when my friend did die of drowning in the ocean at corpus christi. He was out with his older brother far into the water and the undertow took him and he drowned. He was found a day later, leaving the family and friends like me devastated.
