Tell them. No point going on with your life never knowing what could have been. I don't want to live a life in fear of regretting my inaction when I am older, limited to dreaming about what could have been. That's not a life worth living.

It's better to put things out there, and be honest with her/him. Who know's maybe they like you back, and you didn't know all along. Although, I would first be friends with your crush for awhile before actually confessing to him/her that you have a crush on them.

You should never be afraid to tell someone how you feel.

But in doing so it means you'll accept the ramifications of doing so. Meaning you may lose your friend, or wife, or job, etc. It's up to you to determine if those are risks you're willing to take or not.

I was best friends with a group of three people, I started dating one in the group and it got messy from there. We broke up amicably but it did put a strain on our friendship and things were never the same again. I can't say it was worth it but that was MY experience. If we hadn't tried we would always have wished we had. I'd rather go through life gaining experiences, good and bad, then wish I'd done something.
